义工卡

Volunteer Tag


各站站长师兄/师姐

请尽快向欲做义工卡的义工们收集照片、上课证（黄卡）或高级卡 和 马币2零吉， 以便锦添师兄到访贵站时呈交于他。 到访行程表已于日前发出。

谢谢。

制作义工卡必须:-

 1. 呈上初、中级上课证/高级证

 2. 写在上课证/高级证的名字必须与大马卡 (My Card）相同。

 3. 附上一张护照型近照（不超过一年）

 4. 请在照片的背面写上自己的名字和常去做义工的调整站

 没呈上完整资料者，一概不获发出义工卡.

Hi Shixiong, Shijie,

Kindly collect yellow/advance card,photo and RM 2 from your volunteers who wish to do their volunteer name tags. You may pass those collected items to Thiam during his routine visit to your Adjustment Centre (Please refer to the visiting schedule sent earlier).

Please take note :

Compulsory requirements for Volunteer Tag making ：

1. Produce yel low card/advance card

2. Make sure name written on yellow card/advance card is according to I/C.

3. Attach one recent ( Not more than one year) passport size photo.

4. Please write behide your photo , your name and also which adjustment centre you usually go to.

Tag will not be done if the above mentioned requirements have not been given.
